Information we collect
Public forms may collect your name, email address, country, preferred language, request type, a brief description, and your consent to be contacted. Security systems may also process limited technical information needed to protect the website.
- Do not submit medical records, patient names, identification numbers, financial information, photographs, or emergency information.
- AAH does not use public website forms to establish a clinician-patient relationship.
How we use information
AAH uses submitted information to route, review, respond to, document, and safeguard requests; maintain an audit trail; meet legal or organizational obligations; and improve the reliability of the submission process.
Restricted handling
Community support, provider support, and concern reports are classified as restricted. Submission content is encrypted, stored separately from public website content, and available only to authorized administrators. Notification emails contain only a reference number, form type, classification, and timestamp—not the submitter’s identity or narrative.
Sharing and service providers
AAH does not sell personal information. Information may be processed by approved technology providers that support hosting, storage, security, or email delivery, and may be disclosed when required by law or necessary to address a serious safeguarding concern. Access is limited to the purpose for which it is needed.
Retention and redaction
AAH retains submission information only as long as needed for review, response, safeguarding, legal, and recordkeeping purposes. Authorized administrators can redact private content while preserving minimal audit metadata. AAH will approve a formal retention schedule before public launch.
Your choices and questions
You may ask about, correct, or request deletion of information submitted through this website, subject to applicable legal and safeguarding requirements. Use the Contact AAH form for a general privacy question or Report a Concern for a confidential concern.
Emergencies and updates
This website does not provide emergency medical services. AAH may update this policy as operations, laws, or service providers change; the effective date will be revised when material changes are published.
